Financial Assistance

Yuma District Hospital & Clinics has financial assistance available to qualifying applicants.

We also offer assistance with applications for public assistance programs. Yuma District Hospital & Clinics is in full compliance with Senate Bill 12-134 by offering financial assistance to those patients whose income is at or below 250% of the Federal Poverty Level. Yuma District Hospital & Clinics Financial Assistance Program (FAP) 

The FAP has no residency requirements. The Program is solely funded by Yuma District Hospital. Patients are encouraged to apply for the Program, supply all required documentation, and sign applications within 90 days from the date of service. Required documentation includes: Documentation of identification for all members of the household Copies of all income sources Self-employed business ledger or profit and loss statement Copies of personal and business checking and savings bank accounts Income tax statement for the prior year Co-payments range from $10 to $80 for the clinic and 5% to 85% for the hospital depending on the approved rating level assigned.

Bank Loan Program
