• Patient Portal – Register in advance to access records, test results, and communication tools.

Upon Arrival

Please check in at the Front Hub. From there, we will guide you to the appropriate service area:

• Laboratory (Lab)
• Radiology (X-Ray)
• Rehabilitation (Rehab)
• Surgical Services
• Medical Records

Note: You may be asked to complete additional forms. All personal information and medical history will be handled with the highest confidentiality.

Interpreter Services
