Yuma Hospital & Clinics bills all insurance companies on your behalf. To ensure accurate billing, please provide us with a copy of the front and back of your insurance card in one of the following ways:

  • At registration during your appointment

  • Verify your insurance coverage for the services you will receive
  • Understand your plan’s deductible, co-payment, and co-insurance responsibilities
  • Contact your insurance company to confirm benefits and any plan requirements
  • Confirm Yuma Hospital & Clinics is a contracted (in-network) provider
  • For physical therapy: know your insurance’s annual visit or dollar limits
  • Obtain any required pre-certification, prior authorization, or referrals from your primary care physician (if needed for specialist services)
  • Bring your current insurance cards and a government-issued photo ID
  • Notify staff of any changes to your personal or insurance information
  • Be prepared to pay co-payments, coinsurance, or any outstanding balances at check-in
  • You will receive a billing statement after your insurance has processed the claim
  • Payment is due upon receipt of your statement
